to that of the 12 patients who had pretransplant splenectomy (91.6 ± 7.9% and 66.1 ± 13.8% respectively). Posttransplant splenectomy produced the desired effects in all 11 patients in that the leukopenia was permanently reversed, significantly higher doses of cytotoxic drugs were tolerated, and serum creatinine levels declined. Although the numbers are small, this preliminary study is encouraging and indicates that selective posttransplant splenectomy may be a reasonable alternative to routine prophylactic pretransplant splenectomy in recipients of cadaveric renal allografts.

BENEFICIAL EFFECT OF LOW DOSE CYCLOSPORINE WITH MMF (MYCOPHENOLATE MOFETIL) IN RENAL ALLOGRAFT RECIPIENTS
Background: Calcineurin inhibitors (CNI) have significantly reduced the incidence of acute rejection. Nephrotoxicity however may contribute to long-term allograft dysfunction. Mycophenolate mofetil (MMF) may allow cyclosporine (CsA) dose reduction without increasing the risk of rejection. Methods: In seventy-eight living unrelated kidney transplant patients with renal dysfunction, we studi...
متن کاملTHE ROLE OF T-L YMPHOCYTE SUBPOPULATION IN RENAL ALLOGRAFT REJECTION
Twenty-two recipients of HLA-nonidentical living related and nonrelated renal allografts were studied for alterations in the relative percentage of OKT4-positive peripheral blood T-cells after transplantation. Characteristic shifts in the ratio of T-helper to T -suppressor/cytotoxic cells (TH/TS-C), but not absolute cell numbers, were demonstrated to correspond with the status of the allog...
متن کاملRenal Transplant Recipients Treated with Calcineurin-Inhibitors Lack Circulating Immature Transitional CD19+CD24hiCD38hi Regulatory B-Lymphocytes.
BACKGROUND CD19+CD24hiCD38hi transitional immature B-lymphocytes have been demonstrated to play an important role in regulating the alloimmune response in transplant recipients. Here, we analyzed the effect of calcineurin inhibition on these peripherally circulating regulatory B-cells (Breg) in renal transplant recipients receiving cyclosporine A (CsA) or tacrolimus. METHODS PBMCs from health...
متن کاملEarly detection of acute allograft rejection in rat heart transplantation: flowcytometric monitoring of interleukin 2 receptor expression on CD8 positive lymphocytes.
To assess the usefulness of flowcytometric monitoring in the early detection of acute allograft rejection, we studied surface markers of graft infiltrating lymphocytes, coronary sinus blood lymphocytes and peripheral blood lymphocytes after rat heart transplantation. Fisher rats served as donors and Lewis rats as recipients. Among recipients that received no immunosuppression, grafts were remov...
متن کاملMicrochimerism and Renal Transplantation: Doubt Still Persists
Background: The Presence of donor leukocytes in recipients of organ allograft has been shown even several years after transplantation. However, it remains unclear whether this donor cell microchimerism plays an effective role in allograft acceptance or is simply a consequence of immunosuppressive conditions in recipients. Objective: To study microchimerism in a group of kidney transplant recipi...
